The game is set in the same universe as the original, a Japanese university, where the player character assumes the role of a student investigating the mysterious absence of one of his classmates. The same urban legend still circulates about a cursed nameless JRPG game for the TS (a fictional in-game world analogy for the Nintendo DS), which, much like in the previous game, can be transmitted from one portable console to the other. While investigating his classmate's absence, the player discovers a horrible truth about him, and receives a special gift from a mysterious entity: the ability to use his left eye to see things normal humans can't.
Following the original's formula, the real world part of the game is rendered in real-time 3D, controlled through the touch screen input while holding the console vertically, while the "cursed game" part is a 2D JRPG 8-bit style game (reminiscent of the first Final Fantasy), controlled through the d-pad and action buttons while holding the console horizontally, together with new cursed 2D platformer style sections. This time thought the game updates its mechanics a bit, introducing the use of the player's left eye to see threats and ghosts which would be invisible otherwise and to be able to interact with hidden puzzles and games, needed to advance the story. During the progression of the game, the player can also choose which route to take for a given day (only in four days out of seven), one being more challenging than the other.
