Neighbors from Hell is a game with adventure, puzzle and stealth elements. The player controls Woody, a participant in a reality show in which he takes revenge on his neighbor.

The goal of each of the 14 episodes is to entertain the audience by pranking the neighbor. Each episode is themed around a neighbor's activity for the day, and the majority of the "available" pranks need to be successfully pulled to complete the episode. Some pranks can even be chained for a bigger effect and a chance for a small rating bonus. In each episode, the player needs to keep moving around the house and pick up useful objects, as well as follow the movement and activity patterns of the neighbor to deduce what can be combined into a prank.

The player needs to move around the neighbor's house stealthily, because if Woody is caught in the same room as the neighbor, the episode is a failure. Later episodes also require Woody to sneak past the neighbor's pets, and also introduce rooms which are inaccessible in prior episodes.
